Accessible Information for All
The practice aims to ensure that people who have a disability or sensory loss receive information that they can access and understand.
We aim to:
- Ask people if they have any information or communication needs, and find out how to meet their needs.
- Record those needs clearly and in a set way.
- Highlight or flag the person’s file or notes so it is clear that they have information or communication needs and how to meet those needs.
- Share people’s information and communication needs with other providers of NHS and adult social care, when they have consent or permission to do so.
- Take steps to ensure that people receive information which they can access and understand, and receive communication support if they need it.

Online Access to Patient Health Records
You can now view your GP medical record online to look at test results, allergies, immunisations, details of consultations and repeat medication.
Please download the NHS app in order to gain online access. The benefit of this app is you do not need to come into the practice at all to achieve access.
If you would like to have secure online access to your records, we need to make sure that you understand what this involves and that you are happy for us to use the information about you from your application form to set up access on your behalf.
If you decide not to join, or wish to withdraw, it will not affect your treatment in any way. Your request for access may take up to 15 working days to process and longer during peak periods.
We regret the practice cannot assist our patients with this.
Proxy Online Access to a Third Party’s Health Record
Nominated third parties can be granted third party or proxy access to a patient online record. Doctors and other practice staff need to be satisfied that they have the explicit informed consent of the patient to proxy access and follow a formal authorisation process where the patient lacks capacity to consent.
If the patient does not have capacity to consent to grant proxy access and it is considered by the practice to be in the patient’s best interest, Section 1 of the Application for Proxy Online Access to a Third Party’s Health Record may be omitted.
Online access may be granted for parents, carers or legal guardians to access children’s records under the age of 13. Children aged 13 and over would be required to consent to proxy access to their records under General Data Protection Regulations. Proxy access applications will not be accepted from any third party commercial company i.e. Insurance company.
Please ask the practice and then complete the Third Party Consent Form and then visit the practice with a form with ID.
National Diabetic Audit

Data Subject Access Requests
If you require access to your health records, you can request these by asking the practice and then completing the DSAR Application Form.
Once complete, bring this with your ID and supporting documents to surgery reception for us to complete the ID verification and application.
